Employee Benefits at C&N

Your benefits are an important part of your overall compensation. C&N offers a comprehensive array of quality benefits to protect your health, your family, and your way of life. 


Employee benefits provided by C&N


stethoscope icon  Medical Coverage

C&N offers three medical plan options through Highmark® Blue Shield® to allow you and your family flexibility when choosing your health care coverage, with a low-cost PPO2 option to full-time employees and QHDHP free and includes an employer-funded HSA. All plans cover 100% of qualified preventative care costs.

  • Preferred Provider Organization 1 (PPO1): A type of health plan that contracts with medical providers, such as hospitals and doctors, to create a network of participating providers. You pay less if you use providers that belong to the plan’s network. PPO1 has a lower deductible but a higher premium than PPO2.
     
  • Preferred Provider Organization 2 (PPO2): This plan provides the same basic coverage as PPO1, but it has a lower premium and higher deductible and coinsurance.
     
  • Qualified High Deductible Health Plan (QDHP): This plan provides significantly lower premiums and comes with an HSA funded by C&N. You can elect to make additional contributions to your portable HSA account, helping you manage your medical expenses now and in the future.

Each of C&N's medical plans includes prescription drug coverage through Express Scripts, Inc®.

C&N offers two dental options through Guardian Dental® to suit your needs, including individual coverage at zero cost to full-time employees. 

  • Dental Coverage (Per Plan Year)
    • Covers basic and preventative services
  • Premium Dental Coverage (Per Plan Year)
    • Covers basic and prevantative services and partial coverage for major and orthodontia services

C&N offers vision coverage through SIMPLE Vision®. This plan offers you the freedom of flexibility to use the vision provider and service of your choice and acts as a reimbursement program. This program is free to full-time employees and is compatible with other vision plans.


folder icon   Flexible Spending Account (FSA)

Allows pre-tax dollars to be set aside for qualified purposes.

  • HCRA General Purpose: You may set aside pre-tax dollars to pay qualified out-of-pocket expenses not covered by medical, dental or vision insurance plans.
     
  • HCRA Limited Purpose: Similar to a general purpose HCRA, but designed to be compliant with HSA funding by limiting qualified medical reimbursement.
     
  • Dependant Care Reimbursement Account (DCRA): You may contribute a set amount per year to cover eligible dependent care expenses such as daycare.

 

house medical icon   Health Savings Account (HSA)

This account, compatible with a QHDHP, allows pre-tax money (employer or employee) to be set aside and then used (tax-free!) for eligible health care expenses. The account is cumulative and portable!

 


C&N supports employee retirement savings in two ways


money trend up icon   Savings & Retirement 401(k) Plan*

C&N matches your contributions dollar-for-dollar up to 5% of your qualified compensation. You may make these contributions on a pre-tax basis or as Roth contributions and can invest the contributions three different ways:

  • Target Date Funds: Asset allocation is automatically adjusted by the fund manager with the goal of reducing risk as the “target date” approaches.
     
  • Retirement Models: Asset allocation is set by C&N Wealth Management based on individual risk tolerance. Participants change model selection to modify risk.
     
  • Individual Funds: Asset allocation is set and maintained by the plan participant.


chart line up icon   Employee Stock Ownership Plan (ESOP)*

Funded solely by C&N, our Employee Stock Ownership Plan (ESOP) is an employee benefit offering you the unique opportunity to become an owner of the company that you work for. Participate in the success you help build!

The ESOP is a special type of discretionary plan that is designed to make contributions on your behalf in the form of common stock of Citizens & Northern Corporation to a retirement account.

  • Contributions to the ESOP are made annually at the discretion of C&N's Board of Directors.
     
  • Contributions are made based on a percentage of the qualified employee's compensation.
     
  • Generally, to share in any annual ESOP contribution, you must be employed on December 31st and work at least 1,000 hours during the calendar year.

life ring icon   Life and AD&D

C&N recognizes that life insurance provides critical financial protection for your loved ones and peace of mind for you. As a full-time employee, your basic life and accidental death and dismemberment (AD&D) protection are paid in full by C&N.

If a covered employee dies, the basic life insurance policy will pay the surviving beneficiaries a lump-sum benefit.

AD&D insurance pays your beneficiaries an additional benefit equal to the amount of basic life insurance if you die as a result of a covered accident. AD&D benefits are also payable for certain types of physical loss.

 

bed pulse icon   C&N Short-Term Disability

C&N provides all FT employees with self-funded short-term disability coverage.  Benefits are payable after a 30 day waiting period from the first day of absence and provides a benefit of 66 2/3% of your current salary.  You may supplement your disability pay with accrued sick leave up to 100% of your salary.

 

bed pulse icon   Short-Term Disability by State

C&N employees living in covered states are provided additional short-term disability benefit based on qualifying conditions and employment eligibility.  The benefit amount and duration differs by state.

In addition to basic life insurance, full-time employees have the option to purchase additional life insurance on themselves and coverage for their spouse, and dependent children, which is guaranteed issue.

 

bed pulse icon   Short-Term Disability Insurance (GAP)

This policy is designed to coordinate with the 6-week waiting period for employer-sponsored Short-Term Disability. In the event you become too sick or injured to work, you will receive benefits as elected. The money is paid to you directly, and there are no limitations on how it can be spent.

Being hospitalized for illness or injury can happen to anyone, at any time. While medical insurance may cover
hospital bills, it may not cover all the costs associated with a hospital stay. That's where hospital indemnity coverage can help.

Coverage for hospital admission for covered sickness or injury including deductibles and copays, travel to and from the hospital for treatment and childcare service assistance while recovering.

The hospital indemnity plan does NOT include a pre-existing condition limitation, allowing this plan to also be used during admission for maternity and is HSA compatible.

Health coverage is becoming more expensive, with higher co-pays, premiums, and deductibles. Critical illness insurance is an affordable way to supplement and pay for additional expenses that your health insurance doesn't cover.

The plan pays a cash benefit upon initial diagnosis of a covered cancer, with a variety of other benefits payable

Provides you and/or your family with supplemental coverage in the event that cancer is diagnosed
